Fitting your custom lab order into a neat order form isn’t always possible. The important thing is to include all the information that's on our order forms, and then elaborate with your instructions as you see fit. It’s essential that you include your contact information so we can get in touch with any questions. Thanks!

We have THREE types of order forms:

1. Simply print this page and fill it out with a pen or pencil.

2. Download a printable Adobe Acrobat PDF Format ORDER FORM and SHIPPING LABEL This allows you to keep the file handy for future use.

3. Fill out our tabular form-based page and print it from your browser. These forms work best from late model browsers (i.e. not Netscape 40). Many people like the convenience of filling out fields directly from their keyboard and we strive to accommodate them. Frankly, our web designer thinks these forms are inconsistent between browsers and mostly a waste of time, but if you like them please let us know so we can convince him that they are worth the trouble.
